Salman Ahmed Khan, MD
Dr. Salman A. Khan is a dedicated internal medicine and primary care physician with a deep commitment to community health and healthcare equity. Graduating from medical school in Karachi, Pakistan, in 1994, he moved to the United States in 1997 and completed his medical training at LAC+USC Medical Center.
Dr. Khan established his practice in East Los Angeles, later expanded to Van Nuys in San Fernando Valley and San Marino in San Gabriel Valley, where he continues to serve diverse communities. With decades of experience, he has built a strong reputation for providing comprehensive, patient-centered care while actively engaging in health education and mentorship.
Beyond his clinical practice, Dr. Khan is a passionate advocate for healthcare awareness. Through Doc-Talk and collaborations with various medical groups and NGOs, he works to promote preventive health measures and community education. He is also actively involved in mentoring future healthcare professionals through the Shifa Society, guiding aspiring students on their journey into medicine.
Understanding that meaningful change in healthcare requires both clinical expertise and strategic leadership, Dr. Khan is pursuing an Executive Master of Health Administration at USC. He believes that healthcare providers must engage in policymaking to drive systemic improvements, promote health equity, and address social determinants of health.
With his extensive experience, leadership, and commitment to advocacy, Dr. Khan continues to work toward enhancing healthcare delivery and creating a lasting impact in the communities he serves.
Martha E. Saucedo, RN, PHN, MSN, FNP
Martha E. Saucedo, RN, PHN, MSN, FNP, is a highly experienced Family Nurse Practitioner with over three decades of expertise in patient care, nursing leadership, and primary care services. She holds a Master of Science in Nursing with a specialization in Family Nurse Practitioner from the University of Phoenix and has been a dedicated healthcare provider in internal medicine, geriatrics, women’s health, urgent care, and chronic disease management.
Martha has a strong clinical background, having worked in hospital, outpatient, and community healthcare settings, where she has managed acute and chronic illnesses across diverse patient populations. Her role at Primary Care Solutions focuses on providing comprehensive and patient-centered care, emphasizing preventive medicine, health promotion, and disease management.
In addition to her clinical expertise, Martha has held leadership positions in hospital administration, serving as a Nursing Supervisor and ED/ICU Manager, where she played a vital role in patient care quality, staff training, and compliance with regulatory standards. She is bilingual in English and Spanish, allowing her to effectively communicate and connect with a diverse patient population.
Martha remains passionate about advancing healthcare accessibility and ensuring that each patient receives personalized, compassionate, and high-quality medical care.
Salvador Terrones, PA-C
With nearly 30 years of experience in healthcare, Salvador Terrones, PA-C, has dedicated his career to primary care and urgent care medicine. He has worked in diverse clinical settings, including AltaMed Health Services, and private internal medicine practices, providing compassionate and comprehensive patient care.
Salvador earned his Physician Assistant degree from Western University of Health Sciences in 1996, where he was awarded Gold Seals in Surgery and Primary Care Medicine. He also holds a bachelor’s degree in English and Biological Sciences from UC Berkeley.
As a bilingual provider (English & Spanish), Salvador is passionate about serving diverse patient populations and addressing healthcare disparities. He is an active member of the American Academy of Physician Assistants (AAPA), the California Academy of Physician Assistants (CAPA), and the California Shortage Area Minority Program. His extensive experience, dedication, and commitment to quality care make him an asset to the medical community.
Noel Chavez, DMSc, PA-C
Noel Chavez, DMSc, PA-C, is a highly experienced Physician Assistant with over two decades of clinical practice in internal medicine, hospitalist care, family medicine, and psychiatry. He holds a Doctor of Medical Science (DMSc) from Rocky Mountain University and a Master of Health Science from Touro University Worldwide, complementing his extensive clinical expertise with advanced medical education.
Throughout his career, Noel, has served in various hospitalist and primary care settings, working with diverse patient populations across Los Angeles and Southern California. His broad experience spans internal medicine, hospital-based care, primary care, psychiatry, pediatrics, and women’s health, making him a versatile and dedicated healthcare provider.
Mr. Chavez is fluent in Spanish, allowing him to provide culturally competent care to the Latino community. He is certified by the National Commission on Certification of Physician Assistants (NCCPA) and holds multiple professional affiliations, including with the California Academy of Physician Assistants (CAPA).
His passion for education extends beyond patient care—he has previously served as a clinical instructor at Riverside County Regional Medical Center, mentoring future healthcare professionals, and now continued his journey with teaching at Pasadena City College. With a commitment to evidence-based medicine and compassionate care, he continues to make a significant impact in improving patient outcomes and community health.
Rachel Moossighi PA-C
Rachel Moossighi is a dedicated and highly trained Physician Assistant (PA-C) committed to providing comprehensive and patient-centered care. She earned her Master of Physician Assistant Studies from the University of the Pacific and holds a bachelor’s degree in Sociology from UCLA.
Rachel is passionate about preventive care, patient education, and improving healthcare accessibility. Fluent in English and Farsi, with intermediate proficiency in Spanish, Rachel strives to bridge communication gaps and provide compassionate care to diverse patient populations.
She is an active member of the American Academy of Physician Assistants (AAPA) and the California Academy of Physician Assistants (CAPA).

With over 20 years of experience in healthcare, Aurelia Cruz has been a vital part of primary care solutions and a practice manager, managing all three locations with dedication and passion for community well-being. Salman Khan, MD
Dr. Salman Khan is a highly experienced internal medicine specialist with over 25 years of expertise in weight loss, nutrition, and bariatric medicine.
Dr. Khan earned his medical degree from Dow University of Health Sciences in 1995. He later joined the University of Southern California (USC) as a research fellow, contributing to numerous multicenter and NIH-funded research projects. His medical journey began with a surgical internship at LAC+USC, followed by three years of surgical training. This experience sparked his interest in healthcare systems and delivery, leading him to pursue internal medicine.
Dr. Khan successfully completed his residency in Internal Medicine at the renowned University of Southern California. His patient-first philosophy emphasizes collaboration, encouraging patients to actively participate in their care to achieve comprehensive and personalized treatment.
Fluent in English, Spanish, Hindi, and Urdu, Dr. Khan connects easily with a diverse range of patients. Outside of medicine, he enjoys quality time with his family and friends.
Martha Saucedo, NP
Martha Saucedo, FNP, is a dedicated family nurse practitioner with over 34 years of experience in healthcare. She specializes in family medicine, providing compassionate care for patients of all ages, from newborns to seniors. Her primary focus is on preventive care and empowering patients through clear and accessible education.
Martha earned her Master of Science in Nursing and Family Nurse Practitioner degree from the University of Phoenix in 2017. With her Mexican heritage, she is deeply attuned to the needs of the Hispanic community and ensures culturally sensitive care. Fluent in both English and Spanish, she easily connects with a diverse patient population.
Starting her career as a registered nurse, Martha worked her way up to become a family nurse practitioner, consistently striving to deliver the highest quality care. She is a valued member of Dr. Khan’s team, committed to making a positive difference in patients’ lives every day.
